A. Utilize the CE Planning Form to help you ensure you have what you need to submit. The person planning any and all Continuing Education within the ADI needs to be the main point of contact.

B. The person presenting the course needs to complete these three documents. Everything should be copied to the Central office.

  • 1. Affidavit of Image Authenticity
  • 2. Speaker Conflict of Interest Form
  • 3. Speaker's AV Request Form (goes to local arrangements chair)

C. Write up all publicity (advertising) documents for distribution and send to the Central office. Publicity must be informative and not misleading. It must include the following.

  • a. Course title
  • b. A description of course content
  • c. The educational objectives
  • d. A description of teaching methods to be used
  • e. Costs
  • f. The name of the program provider, joint-providers and contact person
  • g. Course instructor(s), their qualifications and any conflict of interest
  • h. Refund and cancellation policies
  • i. Location
  • j. Date
  • k. Specify number of hours awarded by educational methods used (lecture, clinical participation, self-instruction, etc.) and AGD subject code
